Some of the characteristics of US residency programs - blue states (with the exception of California) are ok for IMGs who need visas, red states are less inclined to get IMGs.

IM is not a competitive residency, but ofcourse, it is tough to get into top schools regardless of speciality. A top school not only expects candidates with high 9x percentiles, but also with lot more experience and good recommendation letter. (Ofcourse, here i am only being specific for IMGs). "Emroy" might be tough to get in, but remember there is no gain without pain. (i mean you can try)
