fellowship transition on J1

Hi
Im an IMG and am applying for residency posts this year. I have kept my options open about the visa and have applied for both J1 and H1 sponsoring institutes. My only concern with a J1 is whether I shall have to do the two year tenure/ underserved area work after my residency or does the fellowship tenure get included in the seven years.I have already done my residency in IM in India and my aim is to get into a good clinicl fellwoship. So I dont want a 2 year break in between.

I would also invite candidates in a similar position to mail me so tht we could talk more abt this.
