This quite has lot of questions.

Some hospitals dont prefer candidates without the step 3 before the match to apply for H1b visa.

Some hospitals dont even call candidates if you dont have step 3 and you are looking for H1b visa.

If you PD allows to write step 3 after the match, you are lucky. But only few of them allow such an arrangement. (Generally, the passrate of step 3 without the first year residency is under 60-70%)

H1b visa under premium processing takes 15 days and you should consider additional one month for preparing the documentation for the H1b application.

So if you substract 45 days + 15 days to get the result, that will be 60 days bare minimum.
