i had a few doubts regarding LORS

1) how much weightage are 'waived rights to see ' lors hold over unwaived lors
2)one of my lors does not have a subject line.....is that ok??
3)my waived right lor does not have the aamc id, obviously, so there is no way i can put it on the letter also ( coz its sealed)
so is it ok if it does not have an aamc id?
4)the waived right lor have to be posted from institution or can the sealed package be sent directly to the eras with the whole package?
